First, second, third and fourth dose COVID-19 vaccination appointments, including for the Moderna Spikevax Bivalent vaccine, are available to Conestoga students and employees 18 years of age or older at the Kitchener - Doon campus.
Available through Student Health & Wellness Services, appointments can be booked in advance at the Medical Care Clinic (room 1A102), Tuesday to Friday between 9:00 a.m. and 3:30 p.m. Students can also email medicalcare@conestogac.on.ca or call 519-748-5220, ext. 3679, to book an appointment.
When scheduling your appointment, please ensure you meet the eligibility criteria. Booster doses can be taken at an interval of six months between completion of a primary series or between booster doses. Visit the Government of Ontario to learn more.
Students and employees are required to bring their health card to the appointment or government-issued photo identification, such as a driver's licence or passport if not a resident of Ontario.
Conestoga students who have questions about COVID-19 vaccines are invited to schedule an appointment with a medical provider at the college. Email medicalcare@conestogac.on.ca or call 519-748-5220, ext. 3679, to book an appointment.
International students
International students who received their first or second COVID-19 vaccination outside of Canada must complete the Region of Waterloo Out-of-province COVID Immunization Form and wait for approval before making a vaccination appointment. If you do not reside within the Region of Waterloo, find your local Public Health unit. Vaccination appointments at the Medical Care Clinic can only be made once Public Health issues a valid out-of-province QR certificate.
International students are still able to receive their third and fourth dose at a Regional Vaccine Clinic while waiting for their out-of-province QR certificate.
